Established Patients

At each visit, please notify us of any changes in your address, phone number or insurance information.  Bring your insurance card to each visit.  Please bring all of your medications (prescription and non-prescription medications).

If you are running late, please call our office.  If you will be more than 15 minutes late, you may be asked to wait or to reschedule your appointment.  If you must cancel your appointment, please try to notify us 24 hours in advance.  We wish to be respectful of your time as well as that of other patients.

When the Clinic is Closed

If you have an emergency after office hours, please go to the nearest emergency room or call 911.  Our doctors have hospital privileges at Baylor Medical Center in Garland, Lake Pointe Hospital, Dallas Medical Center, and few others.  If your problem is not an emergency, please leave a message for the appropriate staff person.  Messages will be received on the next business day.  Please do not page the provider on call for non-emergencies such as appointment scheduling, refilling prescriptions, getting test results or referrals, or for billing and insurance questions.  Please call the office during office hours for these matters.

Many insurance plans require a referral/authorization from your primary care physicians before you schedule an appointment with a specialist.  Please call our office when a referral is needed.  An appointment with your doctor may be necessary and authorizations may take 5 days to process through the insurance.

Payment for Services

All co-pays and other payments are due at the time of your appointment.  There will be a $10.00 charge for drawing blood in the office.  This applies to all patients, and must be paid by the patient at the time of service.
